supported housing
|
Who are we? We are a supported housing project which supports 25 people with mental health problems. We work with people who have a range of support needs, and who preferably have some connection with Hackney. Tenants live in single or shared flats on two sites, with an expected length of stay of up to two years before moving on to somewhere more independent. The project is overseen by a Project Manager, with support provided by a Senior Supported Housing Officer and four Supported Housing Officers. We also have a part time administrator who works with staff and tenants. What do we do? We offer housing-related support to all our tenants as well providing a housing management service. This means that, as well as supporting tenants we help sort out rents, maintenance and any other problems with the building. All tenants have a named keyworker, with whom they have weekly keywork sessions. Each tenant has a Support Plan which we will develop together with the tenant. We also will work with each tenant to develop a comprehensive Risk Assessment to ensure that they can safely move towards greater independence. In practical terms the support offered includes advising and working with tenants over any difficulties they may have with:
In aiming to support tenants toward greater independence we offer support in many other ways to ensure the tenant is able to achieve their aspirations. We have regular tenants meetings and an annual survey to enable us to review how well the service is being provided.
